We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team at Healing Mind Plus Wellness Center, located in Baltimore, Maryland. Work Environment

This role is structured as a full-time remote position, allowing for flexibility while maintaining a strong connection with colleagues and patients. The key to our success lies in our collaborative team environment where rehabilitation specialists work closely together. Although primarily remote, there may be occasions requiring in-person meetings or consultations, especially for discharge planning or to review patient progress.

Key Responsibilities

• Conduct thorough psychosocial assessments through patient interviews, observations, and detailed medical record evaluations to determine factors affecting recovery.
• Provide empathetic counseling and psychoeducational support to patients and their families, addressing emotional challenges associated with their rehabilitation journey.
• Work collaboratively with multidisciplinary teams to create tailored treatment plans that integrate psychosocial aspects with physical rehabilitation goals.
• Guide patients in accessing important community resources, including support networks, vocational rehabilitation services, and housing assistance, promoting a seamless integration into society.
• Deliver educational resources for patients and families, empowering them with knowledge regarding post-rehabilitation services and community support options.
• Actively participate in discharge planning processes, facilitating smooth transitions as patients move from our facilities to home or other care settings.

Patient Population / Client Focus

In this role, you will engage with a diverse patient population including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and those experiencing emotional and psychological hurdles following traumatic events. Your expertise will be instrumental in addressing the unique psychosocial needs that arise during their recovery, enhancing their journey toward independence.

Qualifications

• A Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is required.
• Strong communication and interpersonal skills are essential for effective patient interactions.

Preferred Experience

• Experience working in rehabilitation or medical settings is preferred but not mandatory.
• A solid understanding of coping mechanisms, adjustment issues, and grief counseling will enhance your effectiveness in this role.

Compensation and Benefits
