Second Chance Marriage and Family, Inc. is seeking a dedicated Full-time Licensed Clinical Social Worker (LCSW) to join our team remotely, primarily serving our patients at a Hospice Center in beautiful New Castle, Colorado. Work Environment
This is a full-time remote position, allowing you the flexibility to manage your schedule while providing vital services to patients. Although most work will be conducted virtually, occasional evening and weekend hours will be required to accommodate clients' schedules and needs, ensuring that we are available to provide support when our clients need it most.
Key Responsibilities
As a Licensed Clinical Social Worker (LCSW), your primary duties will include:
- Conducting psychosocial assessments and developing comprehensive treatment plans based on individual patient needs and characteristics.
- Implementing evidence-based therapies, including but not limited to solution-focused therapy, behavioral activation, and motivational interviewing, to address a range of mental health challenges.
- Collaborating with multidisciplinary teams, including primary care physicians and psychiatrists, to create an integrated care plan for patients that addresses both mental and physical health concerns.
- Monitoring patient progress meticulously, utilizing appropriate tools and methodologies to evaluate the effectiveness of treatment and make necessary adjustments.
- Providing education and support concerning treatment adherence, medications, side effects, and therapeutic interventions to ensure optimal patient outcomes.
Patient Population / Client Focus
This role is primarily focused on serving patients within a hospice setting, requiring a compassionate approach to support those facing end-of-life issues and their families. You'll be instrumental in guiding patients through emotional challenges, grief, and loss while applying therapeutic techniques that honor their unique circumstances and preferences.
Qualifications
- A Master's degree in Social Work from an accredited institution.
- A current, valid, and unrestricted license to practice as a Licensed Clinical Social Worker (LCSW) in the state of Colorado.
- At least one year of direct experience providing therapy as an independently licensed clinician.
Preferred Experience
While one year of relevant experience is required, candidates with additional experience in hospice care, palliative care, or working with terminally ill patients are highly preferred. Familiarity with telehealth platforms and electronic health records (EHR) is also beneficial, as our practice utilizes these tools for effective patient management.
Compensation and Benefits
The salary for this position ranges from USD 80,000 to 100,000 per year, commensurate with your experience and education. In addition to a competitive salary, we offer the following benefits:
